The point isn't to pre-solve every failure mode, just to have thought through all of them; in practice that ends up pre-empting more problems than expected. Complements the Green/Red Pre-Mortem Question, which applies the same instinct specifically to experiment outcomes.
Before shipping, spend real time enumerating failure modes and scale breakpoints. Once you've thought through them — not solved them all — proceed anyway.
