Quality Assurance (QA) is the function that tests new work against its Acceptance Criteria before it reaches users; when a feature fails that test, the gap is logged as a defect — the standard term for 'when things are broken.' The defect becomes the shared artifact between QA and engineering: QA finds and names it, engineering fixes it, and the feature doesn't move to Production / Launch Milestone until it clears this loop.
